Ultrasound Therapy in Melaka: What It Does, What It Costs, When It Works
Therapeutic ultrasound is the small handheld probe a physiotherapist glides over your skin with a layer of gel. It is one of the oldest electrophysical tools in the clinic, and one of the most over-promised.
This guide explains what it genuinely does, where the evidence is honest, and how a sensible physiotherapist in Melaka actually uses it.
What therapeutic ultrasound really is
The machine sends high-frequency sound waves (typically 1 or 3 MHz) into the tissue through the gel. Those waves create two effects.
The first is thermal: the sound energy is absorbed and warms the tissue a few centimetres below the surface, which can increase local blood flow and make tight tissue more pliable before stretching or hands-on work. The second is non-thermal (sometimes called acoustic micro-streaming), proposed to nudge cell activity during early healing.
It is not the same as a diagnostic ultrasound scan, and it does not "break down" scar tissue or "melt" fat, despite what some marketing suggests. Realistically, it is a warm-up and adjunct, not a cure.
Common claims worth questioning
A few myths circulate widely, including in Melaka. Ultrasound does not "flush out lactic acid", it does not dissolve calcium deposits in any reliable way, and it cannot regrow cartilage or reverse arthritis.
The probe also has to keep moving: holding it still over one spot can overheat tissue and cause harm, which is why a careful physiotherapist never parks it in place. If a practitioner promises dramatic structural change from the machine alone, treat that as a marketing claim rather than a clinical one.

- The physiotherapist assesses your movement and identifies the target area.
- Cool gel is applied; the probe is moved in slow circles for roughly 5–10 minutes.
- You feel mild warmth at most. Sharp pain or burning means the dose is wrong, so speak up.
- The warmed tissue is then used for stretching, mobilisation, or loaded exercise.
That last step matters most. The ultrasound itself is the preamble; the exercise is the treatment.
A reasonable physiotherapist will spend far more of your appointment on assessment, hands-on work, and teaching you what to do at home than on the probe itself.
Where the evidence is strongest and weakest
Be realistic. Systematic reviews of therapeutic ultrasound for common complaints such as chronic low back pain, generic shoulder pain, and plantar fasciitis show small and inconsistent benefits, often no better than a placebo probe.
It is most defensible as a short-term comfort and warm-up measure in subacute soft-tissue problems, used to make the active rehab that follows easier and more tolerable. Some clinicians also use it over small, recently healed areas where gentle warmth helps tolerance of stretching.
It is weakest when sold as a primary treatment over many sessions with no exercise attached. If your only plan is to lie down and be "ultrasounded" twice a week, the science does not support that, and you are paying for passive time that would be better spent loading the tissue.
For most ongoing musculoskeletal problems, the lasting gains come from graded exercise, not the machine.
Who should be cautious
Ultrasound should not be applied over certain areas. Tell your physiotherapist if any of these apply: pregnancy (over or near the abdomen and lower back), an active cancer in the area, a pacemaker or implanted electronic device, deep vein thrombosis, areas with reduced sensation, growth plates in children, the eyes, or over an active infection.
