The honest evidence position: for plantar fasciitis specifically, high-quality studies and Cochrane-type reviews have not shown therapeutic ultrasound to be reliably effective on its own. It may feel soothing and can be a minor part of a session, but it should not be the centrepiece of treatment.
What shockwave therapy (ESWT) does
Extracorporeal shockwave therapy delivers focused acoustic pulses to the painful area, aiming to stimulate a healing response in stubborn tissue. It can be uncomfortable during treatment.
The evidence is stronger here, but conditional: shockwave is generally reserved for chronic plantar fasciitis (typically pain lasting more than three to six months) that has not responded to first-line care. It is not a first treatment, and results vary between people.
